Behavior Modification Agreement
This agreement should be completed when a client has engaged in, or exhibited behaviors (ex., non-compliance with program rules, behavioral issues, etc.) which are of such significant concern that could potentially result in the client’s transfer from the IHP placement. The form should be completed within 72 hours of the incident by the client’s IHP Case Manager. The incident is to be discussed with the client, with the client acknowledging so with their signature. Post the client meeting, the agreement will be reviewed by the IHP site Supervisor, documented, and placed in Client File.
